Terms of Service, Waiver of Liability, Cancellation Policy, and Agreements
Below are the release of liability, agreements, and policies. By purchasing services through Stripe, Venmo, or any other platform, you are agreeing to these terms.
Life Coaching
Acknowledgment and Consent
The client is solely responsible for creating and implementing his/her own physical, mental, and emotional well-being, decisions, choices, actions, and results arising out of or resulting from the coaching relationship and their coaching calls and interactions with the coach. As such, the client agrees that the coach is not, and will not be, liable or responsible for any actions or inaction, or for any direct or indirect result of any services provided by the coach. The client understands that coaching is not psychotherapy and does not substitute for counseling, medical advice, or substance abuse treatment.
Payment
Payment is to be made in full before the start of any coaching session(s) or programs or per the agreed-upon payment plan. If the client chooses to do a payment plan, automatic monthly payments/withdrawals will be required unless other arrangements are made. The due date for the remaining payments will be based on the date of the first invoice unless otherwise discussed. If the invoice has not been paid, sessions will be postponed until paid. The coaching partnership will be terminated if additional due payments are not made within 7 days of invoice receipt/each due date.
Commitment Policy
Choosing a life coach should not be a hasty decision – it’s important to find a good fit. Making sure we’re a good match is important and will greatly benefit both parties, which is why we offer a risk-free consultation call. Due to the amount of time and energy that goes into building programs, providing support, resources, and communications between sessions, administrative tasks, and onboarding and material preparation, I ask that you are fully committed to completing all sessions in your package(s) and doing the work necessary to achieve results. I commit to giving you my full attention, being fully present and grounded to provide the best possible support, and guiding you to the best of my ability. There are no refunds for individual coaching sessions, healing sessions, or programs. Generally, refunds are not offered for packages as well. That being said, my hope is that you know what you're committing to and will fulfill your part of the agreement, but should we decide this is not a good fit or you've achieved your desired results sooner than anticipated, we will discuss the options at that point.
Rescheduling Policy
The client agrees that it is their responsibility to notify the coach at least 24 hours before the scheduled sessions should they need to reschedule. The coach reserves the right to bill the client for a missed session but will attempt in good faith to reschedule the session. If rescheduling often becomes a pattern, the coach maintains the right to terminate the coaching agreement, and the refund policy applies per the above.
Client Responsibility
The client accepts and agrees that they are 100% responsible for their progress and results from life coaching. The coach makes no representations, warranties, or guarantees verbally or in writing. The client understands that because of the nature and extent of coaching, the results experienced by each client may significantly vary. The coach assumes no responsibility for errors or omissions that may appear in any program materials.
Confidentiality
Every session is completely confidential. The coach creates a safe space for brave souls making the big changes necessary to lead a fulfilling life. While the coach is not a mandatory reporter, it is felt that it is a moral and ethical obligation to report suspected abuse or neglect when it is necessary and when there is an indication of serious threat to health and/or safety.
Termination
Either the client or the coach may terminate this Agreement at any time. Please refer to our refund policy regarding any remaining balances
Energy Healing
By submitting payment, I acknowledge that I am voluntarily choosing to receive a Reiki and/or Sound Healing session from Erin Jacobsen (Rebel Mystic, LLC), and understand the following regarding Reiki and Sound Healing practices and potential risks involved:
Understanding of Reiki and Sound Healing:
Reiki is a holistic energy healing practice that involves the practitioner placing their hands gently on or above the client's body to promote relaxation and well-being. Sound healing consists of the use of various instruments to facilitate deep relaxation, stress relief, and energy balancing.
Non-Diagnostic:
Reiki Practitioners and Sound Healing Facilitators do not diagnose medical conditions or provide medical advice; they are not a substitute for medical treatment from a licensed healthcare provider. It is recommended that I see a licensed physician or licensed health care professional for any physical or psychological ailment I may have. I understand that Reiki and Sound Healing can complement any medical or psychological care I may be receiving. I also understand that the body has the ability to heal itself and to do so, complete relaxation is often beneficial. I acknowledge that long-term imbalances in the body sometimes require multiple sessions in order to facilitate the level of relaxation needed by the body to heal itself. I understand that the practitioner will be performing energy healing in my own home, and I am responsible for the safety and well-being of the practitioner while they are in my home.
Energy Healing Potential Risks and Responsibilities:
Emotional Release: During a session, emotions may surface, and I understand that it is my responsibility to communicate any discomfort or concerns to the practitioner.
Physical Sensations: I may experience physical sensations such as warmth, tingling, or pressure during the session, which are considered normal.
Personal Responsibility: I am responsible for informing the practitioner of any current medical conditions, medications, or psychological concerns that may impact my Reiki session.
Waiver of Liability and Release of Claims:
I hereby release Erin Jacobsen (Rebel Mystic, LLC) from any and all claims, demands, or causes of action, whether known or unknown, arising from or related to my Reiki or Sound Healing session, including any potential adverse reactions or discomfort experienced.
Informed Consent:
I affirm that I have read and fully understood the terms of this waiver and freely consent to receive a Reiki and/or Sound Healing session from Erin Jacobsen (Rebel Mystic, LLC).
Confidentiality:
Erin Jacobsen (Rebel Mystic, LLC) commits to maintaining the confidentiality of all information shared during my session.
Cancellation and Refund Policy
I agree to provide at least 48 hours' notice if I need to cancel or reschedule my session. I understand the following: Cancellations must be made up to 48 hours in advance for a full refund. No refunds for cancelations made within 48 hours of my scheduled visit. The appointment can be rescheduled at no additional cost at least 24 hours before the scheduled session. No refunds for completed sessions.
Other
If scheduling a private in-person session, I confirm that I am 18 years or older.
